We would like to welcome you to our practice and thank you for putting your trust in our qualified team.  We are eager to get to know you and want to make your visit as relaxing as possible!  

As with any visit to the doctor’s office you will have some paperwork to complete.  You can find our New Patient Forms and Policies here  or in the navigation to the left; please bring printed copies with you to your appointment to save time.  It is also important that you have a detailed list of all medications and dosages for your record.  If you have any recent x-rays (within the past six months) you may bring them with you or we can request them, with your consent, from any dental provider.

Dr. McCracken will take the time to discuss your medical history, past dental experiences and your goals for your personal oral health and amazing smile!  If you have anything you’d like us to know to make your visit as comfortable as possible, please let us know when you are scheduling your appointment or when you check-in.

Please have your insurance information handy when you call to schedule an appointment or check-in.  We can verify your benefits and be better prepared to maximize every diagnostic benefit that is covered by your plan.

After your first visit Dr. McCracken will detail what is needed, if anything, to keep up your healthy smile!  We will use x-rays, an intraoral camera (to take digital photographs of individual teeth) and education to show you on the computer screen what the doctor is seeing and how you can maintain your oral health goals.

At the conclusion of the appointment our Treatment Coordinator will detail your treatment plan and fully explain your insurance coverage and any out-of-pocket costs.  We will schedule your next appointment at that time.

Amazing Smiles is “in-network” with many providers; if your particular provider is not listed on the insurance page, please call our office as we periodically update our affiliations.  We DO accept all insurance plans and file for your benefits on your behalf regardless of if you are “in-network” or “out-of-network”.
